A 44-year-old Monmouth man died early Monday morning following a workplace incident at the Smithfield Foods facility in Monmouth, Illinois, according to the Warren County Coroner. Patou M. Mpinda died at approximately 2:30 a.m. Monday, authorities said.

The Occupational Safety and Health Administration (OSHA) confirmed Tuesday that it has opened an ongoing investigation into the incident. Smithfield Foods said its safety personnel are working with local law enforcement as officials investigate what happened.

The City of Monmouth expressed condolences to Mpinda’s family, friends, and coworkers following his death. Officials described him as a Monmouth resident who tragically lost his life after the workplace incident.
